1. You are responsible for maintaining the confidentiality of your account and password and for restricting access to your device to prevent unauthorized access to your account.

1.0 Background

Just as your eye-care is important to us, so is your privacy. We do not take your privacy for granted. By accessing this software, you will be required to provide your name, contact, age, country, sex etc.
Eleta Eye Foundation and wiConsultants Nigeria Limited are responsible for releasing this Visual Health Detective application.

2.0 Interpretation

Unless it is otherwise expressly provided or the context otherwise requires, the words stated hereunder shall be construed to have the meaning stated.
"Partners" are wiConsultants Nigeria Limited and Eleta Eye Foundation.
"VHD" means VisionHealthDetective or Vision Health Detective
"The software" means VisionHealthDetective or Vision Health Detective.
"Consent" means a free and uninfluenced decision giving permission.

3.0 Purpose of data collection The Partners have requested for your data for the following purposes;

• 3.1 Conducting Visual Acuity, Contrast Vision and Visual field/surround vision Eye tests via the software.
• 3.2 The processing of results of the tests stated in 3.1
• 3.3 Sending by email the result of the tests stated in paragraph 3.1 above to your email and hospital/doctor earlier stated in the form and Eleta Eye Foundation.
• 3.4 Medical research by the Eleta Eye Foundation without the use of my name.
• 3.5 For follow-up calls, emails and messages by the Eleta Eye Foundation.

4.0 Access to data

• The Partners
• My designated Hospital
• My designated Medical Doctor

5.0 Right to withdraw consent and delete

• 5.1 I am aware and understand that I have the right to withdraw my consent by sending an email to… directing that all my data earlier submitted be withdrawn and same deleted. This shall be a remedy available to the me at any time I feel my privacy has been violated.
• 5.2 Upon sending an email, the partners shall delete my entire data.
• 5.3 A cause of action shall not arise in any court or regulatory tribunal unless this email has first been sent and the partners do not comply.

6.0 Data retention

All the data provided in the form filled will be stored in the Partner’s database for a minimum of twenty years.

7.0 Consent to receive further correspondence

I hereby give my consent to receive emails from the Partners.

8.0 Liability

• 8.1 The Partners shall not be liable for any loss and or damage occasioned as a result of the test conducted using the software or as a result of use of the software. Note further that the software is not a replacement for specialist advice from your physician.
• 8.2 I agree that the Partners can further amend the clauses in this agreement as they wish.
• 8.3 I further agree that in the instance that I use this software for a third party, the Partners shall not be liable for any loss or damage occasioned to the third party.

Having read this agreement, by clicking the “submit” tab below, I understand all the clauses and hereby give my consent particularly the use of my data for the purposes stated in the agreement.
